Our process

Bailey Home Care will assist your family member for as long as they can safely remain in their own home. At that point, we’ll work with you and other Elder care professionals to find an appropriate solution to your family’s care.

Initial Meeting:

During this initial meeting, an assessment is done to identify your needs, requirements, and overall home-situation.The Coordinator will explain Bailey Home Care’s in-home care services and suggests the level of service deemed most appropriate. Most often the discussion leads to a mutually acceptable Plan of Care.

Plan of Care:

The Plan of Care is a document outlining the specific care services, schedules, client preferences, and special instructions, which Bailey Home Care employs to achieve your satisfaction. As your situation changes, the Plan of Care is modified to reflect those needs. It provides a central focus to all parties involved with your care.

Introducing the Caregiver:

Once the Plan of Care is accepted and a service agreement is completed, we guarantee to match you or your family member with just the right caregiver based upon personalities, skills, expertise, interests and attitudes.

Continued Follow-Up:

After the aide has been placed in the home, Bailey Home Care Coordinators continue to make weekly visits and telephone check-ins to you and your assigned aide to ensure the best possible service is being provided. In addition, a dailey Care Log is kept by the aide for the family’s review. The Care Log allows you to see what was done when, where and how during the aide’s stay.
